The description below is excellent save one point...
In the end, it seems that Wataru is the only one who did NOT know that Mami wasn't a sister. All the others had it figured out by that time. He only figured it out when she told him point blank, and even then, he told her to "stop kidding around" the first time she said so.

Wataru Minakama was a high school student who literally looked like everyone else and made a promise with his friend Aiko to become a very respected member of the community, but got every answer on the final test wrong and his dream went plummeting down. He got a surprise when his fathers' butler Jeeves told him his history and took him to Promise Island, then left him there. Poor Wataru. But then he met his four sisters Karen, Sakuya, Hinako and Kaho, then met the rest of his sisters the next day in his new home, the Welcome House. When the spying Mami was trying to leave the Welcome House Wataru instantly mistook her for his thirteenth sister(apparently apart from Yamada, he was the only one who knew she wasn't related to him).
Wataru is several different things to his sisters; to Kaho, Hinako and Aria he is someone fun to play with. To Mamoru, Rinrin, Marie, Shirayuki and Yotsuba he's the older brother they always wanted and the final quartet of Karen, Sakuya, Haruka and Chikage are undeniably attracted to him and want to be something more than sisters to him, maybe because he's the only boy around them all the time. He runs into Jeeves in disguise, none of which fool him. Wataru wasn't too pleased about having so many sisters in the start mainly because they wouldn't leave him alone on his first day of school but soon they grew on him.
The names he's refered to;
"Dear Brother" Sakuya.
"Beloved Brother" Haruka.
"Mon Frere" Aria.
"Brother Mine" Marie.
"Bro Bro" Hinako "Hina".
"Bro" Rinrin.
"Big Bro" Mamoru.
"Brother Darling" Chikage.
"Brother Dearest" Yotsuba.
"Brother" Kaho.
"Elder Brother" Karen.
"Bud" and later "Older Brother" Mami.
He has a concerning and kind nature that causes his sisters to absolutely adore him even more than they already did. Still, he can't help but recognize that he is a shiroteen, his preteen sisters can sometimes distract him without knowing it and it seems as though the younger sisters, Kaho, Rinrin, Shirayuki and Mamoru seem to be using him as a test subject to see how much potenial they might have at being a girlfriend to someone.

Start off as a total bookworm, who gets good grades but has minimal social life. After he accidentally failed an enterance exam, Wataru found out that he's already enrolled to another school, located on the "Promise Island", and subsequently sent there. On the island, Wataru met 4 very cute girls who claimed to be his younger sisters, next day the rest shows up, raised the total figure to 12. Live with his siblings in a big house, Wataru has to contend with the affections of his siblings, as well as helping them in their daily lives, and rediscover happiness with family.
- that's his description for the 1st TV series.
In Re-pure, he's simply known as 'Onee-chan' (or in other 11 variations of it), though not living together with the girls, he still spend times with them from time to time.
